Established in 1954 as the oldest medical college of Khyber Pakhtunkhwa, an initiative of Monowar Khan Afridi in collaboration with provincial Chief Minister Sardar Abdul Rashid. Governor-General Ghulam Muhammad laid the foundation stone on 2 May 1954, and the first batch of 60 students began classes in September 1955. The college separated administratively from the University of Peshawar in 1975 and now functions with institutional autonomy under Khyber Medical University.

Peshawar is one of South Asia's oldest continuously inhabited cities, historically the gateway to the Khyber Pass and a major centre of Gandhara civilization, long serving as the political and cultural capital of Khyber Pakhtunkhwa.
